To successfully complete your application, please bring:

• Completed DS-11 New Passport Application – do not sign the application

• Completed DS-82 Renewal Application - do not sign the application

• One Passport Photo (we can take your photo! - $15.00 charge)

• Certified copy of birth certificate, Certificate of Naturalization, or expired passport

• Photo ID –Drivers’ License, State ID, Military ID

• Payment in check or money order only

• If passport is for a child (under age 16), both parents must be present
